Rosacea (erythematotelangiectatic type)

Facial vascular hyperreactivity manifesting as transient or persistent redness (flushing), caused by dysfunction of the innate immune system and neurovascular regulation.

Description: A chronic inflammatory and neurovascular disease affecting the face (forehead, nose, cheeks, chin) exclusively. It is characterised by episodes of redness and heat (flushing), persistent redness (erythema), dilated capillaries (telangiectasias), inflammatory lesions (papulopustular rosacea), and, in later stages, tissue thickening (phymatous changes, e.g., rhinophyma).

Causes: It arises from innate immune hypersensitivity (activation of TLR-2 receptors and excessive production of the pro-inflammatory cathelicidin LL-37) and neurovascular sensitivity (TRPV1 channels on nerves overreact and release neuropeptides that dilate blood vessels and promote inflammation). Excessive concentrations of Demodex mites in the follicles are also common. Key triggers include UV radiation, stress, temperature changes, alcohol (especially red wine), spicy food, and foods rich in histamine (tomatoes, chocolate, citrus fruits).

Best management strategies:

Protection and care: Strictly avoid triggers, cleanse only with gentle products, and use daily protection with mineral UV filters (zinc oxide and titanium dioxide, which do not generate heat on the skin).

Topical treatments: Ivermectin 1% cream (which eliminates Demodex and reduces inflammation), azelaic acid, metronidazole, or microencapsulated benzoyl peroxide are used for pimples. Topical vasoconstrictors (brimonidine, oxymetazoline) are used to reduce redness.
